This form must be completed by August 16, 2024 and accompanied by completing your payment(s) for the Academy before the Academy school year begins.

This grade must match the grade level in the secular school unless you have made other arrangements with the Academy Principal.

Note: There is a $50 early bird discount if you register by May 31, 2024!
9th-12th graders who wish to assist in the classroom can choose to either get paid or use these hours as volunteer credit.
If yes, we would love to sit down with you before the school year begins so that we are able to discuss the best ways to support your child from the outset. Please contact our Academy Principal to schedule an appointment.

Section C: Release and Learning Commitment Forms

Participation Release: By signing my name below, my child has permission to participate in the Beth Hillel Bnai Emunah Academy. In consideration of my child's acceptance as a religious school student, I hereby waive any and all claims against Beth Hillel Bnai Emunah, its agents and its employees that may arise out of any injury, loss or damage suffered by my child during any religious school activity. I hereby authorize the Academy Principal, or person designated by the Academy Principal, to obtain emergency medical care for my child in the event such care is indicated. I give my permission for my child to receive emergency medical care by any nurse, doctor, paramedic or member of a medical staff of a hospital licensed by the State of Illinois. I understand that every effort will be made to notify a parent/guardian prior to treatment.

I certify that my child is in good physical health. My child has my permission to participate in all activities that are part of the regular religious school program.

Learning Commitment: At BHBE we are committed to Lifelong Jewish learning and aim to provide all students this foundation in childhood.  In order to become a B-Mitzvah at BHBE, we expect that students will attend a meaningful Jewish educational program in order to achieve this goal.  Participation in the BHBE Academy twice a week from 3rd through 7th grade fulfills this requirement. Students are highly encouraged to attend through 8th grade and some or all of high school as well.

Media Release: From time to time your child’s photo may be taken in our classrooms or special events.  We use these photos in the synagogue newsletter, on our synagogue display boards, the BHBE website as well as private Facebook groups and other social media and publicity materials. I give permission for Beth Hillel Bnai Emunah to use pictures taken of my child during school activities for publicity both internally and externally. Photos will not include your child's name unless explicit parent's/guardian's permission is granted.

We are creating a system by which our volunteer parents can reach out to all parents in a classroom in the event of a school closing, programming and other important information that occurs at the BHBE Academy. Your permission here allows us to reach out to you in this way.
Section D: Congregation Community

You may have noticed that at the top of the form we begin with family information.  This is intentional.  The work of raising and educating Jewish children cannot be done in the school alone, but is a project of the school, the family and the community. You are not just registering your child for school today, but in fact your entire family.  When you come in the building and spend time learning and volunteering with us, you make a powerful statement to your child that Jewish learning is a lifelong process and that we are all on the same team in creating these experiences for them.  Don’t underestimate the subtle power of this statement!

There are days on the calendar when parents are expected to attend with students, as well as days they are encouraged to do so.

In addition to those opportunities to participate we are in need of regular volunteers to make our school work.  Would you consider supporting our school in an ongoing role or a limited engagement?

Section E: Payment Information

In Jewish tradition, it is the job not only of parents but of the community to provide education for children. The BHBE community is committed to this obligation.  As such, our Academy tuition only covers a portion of the cost of our religious school.  Both fundraising efforts like the Purim Carnival and congregational dues heavily subsidize our children’s education.  We are best able to plan appropriately when we know early on how many students to expect in each grade.  As an incentive for you to register early, we offer a $50 early bird discount to families who register on or before July 1. (The early bird discount code will be found on the payment screen.) Regular fees go into effect on July 2.

To receive the early bird credit, type EARLYBIRD in the discount field on the next screen, and then click anywhere outside the field. (This $50 family discount will be available through July 1.) You will also be able to pay by electronic check or credit card, set up payments plans and more. A convenience fee is added for credit card payments, but not for ACH -- electronic checks. Payment plans can be set up with either option.
